The majority of the unemployment insurance companies we use now accept Statement of Fact instead of a paper application form. This means that a signed application form is no longer required. After completing your application over the phone, or online, you will receive a copy of the information upon which the application is based. You must advise us immediately if any of the information on the Statement of Fact is incorrect.
